Dr. Carney’s approach to lower eyelid surgery is referred to as transconjunctival: making a slight incision in the lower lid and removing any of the existing three fat pads. This surgery is quick because there is no suturing. It offers fewer risks, less bruising and swelling, and a more rapid recovery.

Dr. Carney encourages lower blepharoplasty be followed by a skin tightening procedure or a chemical peel to improve skin laxity. In these cases, his standard approach is to remove the fat pads, and follow with a TCA, 88% phenol, or Baker-Gordon peel depending on the laxity of the tissue.

transcutaneous blepharoplasty involves an incision on the skin's surface, just under the lower lash line, and is also an effective approach in removing fat pads creating under eye fullness, as well as an option to remove excess skin.

To correct upper eyelid drooping, a very standard elliptical shaped excision is made in the lid and both skin and the orbicularis oculi muscle are removed. The nasal and medial fat pads are then removed. The incision is closed and repaired using Prolene sutures that are removed five days after the procedure.
